Bluecard Policy
It is a requirement of the Point Lookout Surf Life Saving Club and Surf Life Saving Qld for all members
"over the age of 18 who work with Children or Young People" to complete a Blue Card Working with Children Check.
As parents travelling with Junior Activities you are seen to be "working with Children and Young People", and as
such are required to make application for your Blue Card.
The Blue Card Working with Children Check is confirmed by the Commission for Children
and Young People and Child Guardian. This process is confidential and assists the Club
in our duty of care for providing a safe environment for your children.
All forms are available by selecting the Forms option on the side menu.
Please complete the form that best meets you situation as described below:
- If you do not have a current Blue Card download the Blue Card Volunteer Application Form
- If you have a Blue Card issued through your current employer or other sporting
organisation, download the SLSQ Authorisation Confirmation Form

If you need to renew your Blue Card download the Blue Card Renewal Form

Note:
- There is no charge for the processing of your card through the Surf Club.
- Please do not forward forms directly to the Commission or Surf Life Saving Qld
as the Club Administrator needs to process them into the club administration system prior to submission.
- When handing your form in, please ensure that you have a adequate identification
as per page 2 of the application form.
